For health systems

Support patients between visits

Explore diabetes, blood-pressure and weight-management programs that complement your care team and help patients stay engaged in their care plan.

Dario connected devices: a blood-pressure monitor, a smart scale, a smartphone glucose meter and app screens with sample readings.
Program options

Two programs to scope with your care team

Dario's digital programs run today for employers and health plans. Health-system programs are new: the options below are proposals to scope with your team as a pilot.

Option A

Between-visit cardiometabolic support

  • Connected monitoring at home
  • Digital self-management support
  • Coaching suited to the selected program
  • A workflow scoped with your care team

Clinical decisions stay with your clinicians or a provider arranged for the program.

Option B

Follow-up and selected care-task support

  • Helping patients follow agreed next steps
  • Supporting navigation to appointments, tests or existing services, where a workflow is set up
  • Agreeing how completion is documented and measured

Which tasks are supported, and how, is agreed for each program.

Where a pilot could help

Between appointments

A pilot could give patients with diabetes, high blood pressure or excess weight daily support at home, through a workflow agreed with your team.

Follow-through on care tasks

A pilot could help patients follow the next steps your clinicians agree, such as a follow-up visit or a test, with completion measured separately.

Reconnecting with your care team

Explore ways to support follow-through on referrals and reconnect patients with your care team.

Measure each step separately

A pilot agrees which of these steps it will measure. A reminder or a delivered device does not by itself show that a visit happened or a care gap closed.

  1. Engagement

Patients enroll and use the program

  2. Referral

A next step is offered, such as a prompt to book a visit

  3. Completed service

The visit, test or service takes place

  4. Accepted documentation

Your team accepts the record for the chart or quality reporting

  5. Clinical outcome

A clinical measure changes, such as blood pressure

Start with a scoped pilot

A pilot is agreed before launch, with:

  1. A defined patient group
  2. A named clinical sponsor and budget owner
  3. Agreed responsibilities for your team and Dario
  4. Data, consent and privacy requirements
  5. Specific success measures
  6. A commercial proposal

Partner clinical services can be considered where they fit the program: see clinical care options.

Evidence from Dario's earlier diabetes solution

Two peer-reviewed, Sanofi-funded observational studies found better lab HbA1c results and lower hospitalization among adults with type 2 diabetes using Dario's diabetes solution in 2017 to 2021 than among matched non-users. They do not measure today's program.

Neither study evaluated a health-system program.
