Help members manage diabetes, blood pressure and weight between visits
Support members between visits with connected monitoring, coaching and reporting on participation and clinical measures.
The same member program, with clinical care that differs by line of business
Program components are selected for your population and agreement. Devices and support depend on the member’s enrolled program.
Members get the devices their program uses, such as a glucose meter that plugs into the phone (with strips and lancets), a connected blood-pressure monitor or a scale, plus the Dario app and coaching by certified diabetes educators and other specialists, by program. You get enrollment, participation and clinical reporting. Reporting cadence is agreed during implementation.
| Line of business | Digital program and coaching | Partner clinical care, such as GLP-1 prescribing |
|---|---|---|
| Commercial and ASO | Offered | Available through Dario's licensed provider partners, subject to state and contract |
| Medicare Advantage | Offered | Not yet confirmed; assessed for the specific plan, state, provider and contract |
| Medicaid | Offered | Not currently available. Members can be referred to the plan's own network |
You choose who is invited
Offer Dario to fully insured members, self-funded groups, or a population you select from claims. Enrollment confirms eligibility using the process you agree. Billing arrangements are confirmed during contracting.
How one plan offers it. Sentara Health Plans' member page says it offers Dario's year-long type 2 diabetes program to selected adult Virginia Medicaid and D-SNP members. Sentara selects members using clinical criteria and claims data and invites them by mail or email; members enroll by phone or online and receive a glucose meter, supplies, the app and diabetes-educator coaching. The page reports no participation or outcome results. Sentara's member page
What it adds for the plan
Support between visits
Readings log automatically from connected devices; members get lessons, coaching and shareable reports.
Evaluation in your claims data
With shared claims and an agreed method, utilization and cost can be compared with matched non-participants: differences, not guaranteed savings, and claims miss some clinical outcomes.
The evidence behind it
Two peer-reviewed, Sanofi-funded observational studies found better lab HbA1c results and lower hospitalization among adults with type 2 diabetes using Dario's diabetes solution in 2017 to 2021 than among matched non-users. They do not measure today's program.
How Dario reaches your members
| Step | Your organization | Dario | Channel partner, if you contract through one |
|---|---|---|---|
| Contract | Contracts directly or through a channel partner | Scopes programs, clinical pathways and reporting | May hold the contract and handle billing |
| Eligibility | Sends an eligibility file, or enables single sign-on | Tests the file or sets up sign-on | May supply or route eligibility data |
| Outreach and enrollment | Agrees the outreach plan; can promote it in your channels | Provides materials and runs the agreed outreach | May run outreach to the people it serves |
| Activation | Not typically involved | Ships devices, onboards members and starts coaching | Not typically involved |
| Reporting | Shares claims data for an economic analysis, if wanted | Reports enrollment, participation and clinical measures at the agreed cadence | May receive reporting under its own contract |
Enrollment depends on outreach; Dario does not guarantee a participation rate.
